Finalize Paperwork

For the best experience, please fill out your paperwork at least 24 hours before your appointment. NOTE: New client paperwork MUST be finished before you can begin your visit. If you have not completed your new client intake paperwork, please follow this link to do so now.

Pay A Bill

If you have a co-pay or bill, you can always pay this through your Patient Portal. (Note: your invite to the patient portal was sent through email and expires within 72 hours. If you need us to resend this invitation, please give us a call at 270-310-9200

Do you have an upcoming Telehealth Appointment?

When you are ready to start your visit, click on “begin telehealth” on the app and choose your provider’s name. Then you’ll be added to our virtual waiting room.
